In order for Wuesthoff Health System to better inform Brevard County healthcare consumers on hospital pricing and "transparency," the hospital system is requesting Explanation of Benefit (EOB) health insurance documents from Brevard County residents. We are gathering EOB forms - with confidential information removed - for many types of procedures in numerous categories including:

• Appendectomies
• Colonoscopies
• Emergency department visits
• Cardiac procedures
• Gall bladder procedures
• Gynecological procedures
• Joint replacement surgeries
• Mammographies
• MRI/CT scans
• Obstetric procedures
• Spinal fusions
• X-Rays

We are only looking for Explanation of Benefit forms from:

• Aetna
• BlueCross Blue Shield
• Cigna
• Health First Health Plans
• United Health Care

The first person who submits an Explanation of Benefit (EOB) form that qualifies from each category listed above will receive a $100 gas card, to a maximum of three gas cards per person.
